Is Getting Ketamine Covered By Insurance Possible With Off

Study: Ketamine therapy improves depression & anxiety

While it is possible to get ketamine covered by insurance with it remaining off-label for mood disorders and pain conditions, there are many other hurdles to overcome. One of them is pharmaceutical companies do not want to see ketamine used therapeutically because it could reduce their sales of traditional antidepressants, which are highly profitable for them despite being less than half as effective. Insurance companies typically resist accepting ketamine as a credible treatment for depression, other mood disorders, and chronic pain because it is an off-label use. Generally, they tend to deny coverage of treatments whenever possible in an attempt to save money. At KCLA, we think they would actually save money if they covered ketamine instead of some of the less effective treatments they currently cover, many of which cost more than ketamine infusions.

Fortunately, there is an increasing awareness developing in the medical and psychiatric communities where ketamine therapy is acknowledged as a crucial procedure that needs to be mainstream. The United States and the rest of the world has an urgent need for solutions to the depression and suicide crisis. Depression is the leading cause of disability in the US and worldwide.

With the recent FDA approval of Spravato AKA esketamine for depression treatment, there is an increased chance that insurers may consider offering coverage for other ketamine treatments, such as Intravenous Ketamine Infusions.

How Much Does Spravato Cost Without Insurance

Without Medicare Spravato coverage, the following are average costs for the treatment. These costs will differ by location and other variables. Therefore, they may not reflect the actual cost of what youll pay for this treatment in Tennessee.

Youll most likely pay more at the beginning of treatment and less in the long run. Depending on whether youre receiving Spravato doses weekly or bi-weekly, costs can range from $5,664 to $8,142 in the first month of treatment, $2,832 to $4,248 in the second month, and possibly $1,416 to $4,248 each following month.

Why We Dont Take Insurance:

We are happy to give patients advice and support and help and superbills to take to their insurance companies. The constant appeals and denials and run-around that is standard with insurance companies have driven many doctors offices out of business. We arent their customers and insurance companies dont have to go out of their way to make us satisfied and whole. Many of our patients already know that mental health as a whole is very poorly reimbursed by insurance and many providers have chosen to leave in-network coverage rather than continue to endlessly fight for fair payment.

It remains the case that insurers dont pay for ketamine treatments, with exceptions proving the rule. We cannot take insurance because we would not be reimbursed.

Possibly Maybe Getting Some Reimbursement:

Since we opened Lone Star Infusion in 2015, we have slowly begun to see a little movement in patient reimbursement by their insurers. While the insurers dont have policies to cover ketamine treatments, and while they will rarely reimburse the full cost of a treatment, some patients may be able to collect some money from their insurance company with a lot of work.

Each insurance company does this a little differently. Often different patients will even see different results from the same insurer. Some patients may get the expense applied to deductibles, some patients may get the office visit covered, and there are a few patients who are say they are being reimbursed by their insurance for the entire amount of their treatments.

I wish it was more uniform and I wish it didnt take so much follow up and homework on the patients side. But insurance companies dont have to cover something just because it has FDA approval. They dont have to cover it at a reasonable price. They dont have to cover generic ketamine even when a less-effective derivative of ketamine got FDA approval.

Ketamine In The Treatment Of Sickle Cell Vaso

Hagedorn and Monico noted that sickle cell disease is the most common hemoglobin variant in the world and can present with recurrent painful vaso-occlusive crises . Pain control in these patients is often difficult and requires a multi-modal approach. In patients with uncontrolled pain after traditional pain control methods, ketamine infusions have been used. Unfortunately, no source alone had described ketamine infusions for pediatric patients with SCD. These investigators provided a thorough definitive reference for health care providers regarding the use of ketamine infusion for pain control in pediatric painful VOC. Low-dose ketamine infusions for pediatric patients with SCD painful crises resulted in improved pain scores and reduced opioid usage. The authors concluded that ketamine infusions have shown promise in aiding physicians in appropriately treating pediatric sickle cell painful crises.

In a review on Integrative approaches to treating pain in sickle cell disease , ketamine is not mentioned as a therapeutic option.

An UpToDate review on Acute vaso-occlusive pain management in sickle cell disease states that Ketamine may be appropriate in individuals with severe pain not responsive to standard opioid analgesics Ketamine should only be used by programs experienced and trained with its use and following a standardized protocol. However, ketamine is not listed in the Summary and Recommendations section of this UTF review.

Ketamine In The Treatment Of Substance Use Disorders

Ivan Ezquerra-Romano and associates examined the pre-clinical and clinical research into ketamine’s ability to treat addiction. Despite methodological limitations and the relative infancy of the field, results thus far are promising. Ketamine has been shown to effectively prolong abstinence from alcohol and heroin in detoxified alcoholics and heroin dependent individuals, respectively. Moreover, ketamine reduced craving for and self-administration of cocaine in non-treatment seeking cocaine users. However, further RCTs are urgently needed to confirm ketamine’s efficacy. Possible mechanisms by which ketamine may work within addiction include: enhancement of neuroplasticity and neurogenesis, disruption of relevant functional neural networks, treating depressive symptoms, blocking reconsolidation of drug-related memories, provoking mystical experiences and enhancing psychological therapy efficacy. Identifying the mechanisms by which ketamine exerts its therapeutic effects in addiction, from the many possible candidates, is crucial for advancing this treatment and may have broader implications understanding other psychedelic therapies. The authors concluded that ketamine showed great promise as a treatment for various addictions, but well-controlled research is urgently needed.

    What About Private Insurance

    Ketamine Treatment Q& A | Kyle Kittleson x MedCircle

Most private insurance companies will only cover ketamine as an anesthetic, which means ketamine therapy does not qualify. However, there are other parts of the process, such as visits with the doctor and medical monitoring, which may be covered. We will work with your insurance company to make your treatment as affordable as possible.

What Does Medicare Cover For Depression

While Original Medicare is unlikely to cover ketamine infusion for depression, it does provide coverage of various other depression treatments.

    • Medicare Part B covers one depression screening per year. Part B also covers other mental health care treatments received in an outpatient setting, such as therapy, family counseling and diagnostic testing.
    • Inpatient mental health care is covered by Medicare Part A. This includes any mental health treatment you receive as an inpatient at a general hospital or at a hospital that specifically offers mental health treatment.

So Why Doesn’t The Fda Just Approve Ketamine For Depression

The FDA approval process is very expensive and time-consuming. As a point of reference, the average time from FDA application to approval is 12 years, and the estimated average cost of taking a new drug from concept to market exceeds $1 billion. Since ketamine is already an approved drug with substantial safety and efficacy data, ketamine insurance coverage would likely be faster and less costly, but could still easily exceed tens or hundreds of millions of dollars and years of additional research. Since the patent is up on ketamine and no one company can recoup that cost, and it is already in use off-label, it isnt likely to be approved anytime soon. This is the case for many other medications that are used off-label. As a result, health insurance companies can deny coverage for ketamine infusions for the treatment of depression, other mood disorders, and chronic pain despite their efficacy. Unfortunately, the FDA doesn’t approve medicines and insurance companies don’t pay for them just because they can help people.

Why Many Insurance Companies Dont Cover Ketamine Therapy

Insurance companies have stated a few reasons for lack of coverage, one of which is that ketamine therapy is prescribed as off-label.

Off-label prescribing is when a clinician prescribes a medication in a different way than was explicitly approved by the FDA. Because ketamine was approved by the FDA in 1970 as an anesthetic, but has not been re-approved by the FDA specifically for treatment of depression and anxiety, some insurers choose not to cover it. However, ketamine therapy has been used to treat mental health conditions for more than 20 years.

There has been progress made towards more FDA approvals. For example, Esketamine was approved by the FDA in 2019 for patients with treatment-resistant depression. Though not all insurance companies cover Spravato, it is increasing in popularity and slowly gaining more coverage.

Furthermore, research is currently promoting multiple psychedelic medicines to be granted FDA approval. This includes generic ketamines application in treating a number of conditions. There is hope that effective treatments like ketamine therapy will be more widely covered as FDA approvals continue to roll out.

Is Ketamine Therapy Covered By Your Insurance Provider

Though coverage of ketamine treatment is limited, there are some options to help determine if your insurance provider will cover ketamine treatment. Heres some steps you can take:

  • Contact your insurance provider
  • Contact your insurance provider before starting ketamine therapy to ask if they provide any type of coverage to help reduce or cover the cost of your ketamine treatment.

Insurance coverage for ketamine therapy varies on a case-by-case basis with some providers willing to cover partial up-front costs of ketamine treatment. For example, a treatment can be designated as an infusion of a generic drug for partial coverage. Discussing these options with an insurance representative ahead of time can make a big difference in your available coverage.

After receiving care, your insurance provider will send an explanation of benefits, which details the costs of treatment and how much insurance will cover. In some cases, youll need to meet your deductible before the insurance provider will provide full or partial coverage of the cost of any services. This is another reason why its important to discuss your options with a knowledgeable representative.

  • Discuss out-of-network reimbursement
  • Out of network reimbursement is when your insurance provider asks you to pay your clinical provider out-of-pocket, or using your own finances, with the intent of reimbursing you.

  • Provide a superbill, if requested

Mostly no. The reimbursement for outpatient ketamine infusions are all over the place. The sad truth is that a majority of insurance companies are not covering outpatient IV ketamine treatments. The biggest challenge is that ketamine infusions for the treatment of mood disorders like depression are not FDA approved . Therefore the insurance companies argue ketamine treatments are too experimental and that it shouldn’t be covered.

Things are s l o w l y changing as some health insurance companies realize the benefits of off-label ketamine use.
