In the 116th Congress, the House of Representatives passed the Elijah E. Cummings Lower Drug Costs Now Act , which among many provisions, provided for dental coverage under Part B of the Medicare program, starting in 2025 if enacted into law. Medicare would cover 80% of the cost of preventive services and basic treatments, and would phase up coverage for more major treatments, covering 10% of the cost in 2025, increasing 10 percentage points per year to 50% in 2029 and thereafter. The Congressional Budget Office estimated this benefit would cost almost $238 billion over 10 years . The recently reintroduced version of H.R. 3 does not include coverage for a dental benefit.

Representative Doggett introduced the Medicare Dental, Vision, and Hearing Benefit Act in July 2021 that would provide dental coverage under Medicare Part B. In contrast to H.R.3, the proposal, according to a press release, would require no coinsurance for preventive services, and impose a 20% coinsurance for more extensive benefits, which is similar to how the Part B benefit is structured for physician and other outpatient services, and is lower than the 50% coinsurance typically charged by Medicare Advantage plans.

Medicare Part B Dental Benefits

On the other hand, if the physician conducts the examination needed prior to kidney transplant or heart valve replacement, the CMS states that Part B benefits will apply.

However, when it comes to Medicare Part B, there are two specific sets of services that it will not cover.

The first involves services used to care, treat, remove, or replace teeth to structures supporting the teeth. For example, this can include pulling teeth prior to getting dentures.

The second set of services Medicare Part B wont cover also include those related to the teeth and their supporting structures, unless those services are needed to effectively treat a non-dental condition.

In this type of situation, the dental service must be performed at the same time as the covered service in order for Medicare to pay its portion. It must also be performed by the same healthcare professional who performed the covered service, whether that person is a physician or dentist.

An abscess is an infection in your gum or at the root of a tooth. If you develop an abscess, your dentist may perform a root canal treatment or pull the tooth. The dentist may also prescribe antibiotics. Many dental policies cover root canal treatments, but youll have to pay part of the cost.

Your Part D plan should pay for antibiotics your dentist prescribes. If the infection spreads and becomes more severe, Part B will cover a doctors care, and Part A will cover hospitalization.

Medicare Advantage plans with supplemental dental coverage that includes dental implants will be most helpful. You may have to pay an additional monthly premium, and there will always be a maximum dental benefit amount allowed, along with either a copay or coinsurance. If you need dental implants, you will incur some amount of out-of-pocket expense.

Not all MA plans cover dental implants. Some MA plans specifically exclude dental implants, even if they offer other comprehensive dental benefits.

MA plans list dental implants as either a covered item or an exclusion in the Evidence of Coverage document associated with each plan. Medigap plans do not cover dental insurance or copayments. They are a way to supplement Medicare costs. As Medicare does not provide dental benefits, Medigap does not help a person fund these.

Clinicians have observed an association between oral disease and endocarditis, and describes a causal model offering a possible explanation .

The oral cavity harbors a lot of bacteria, most commonly in the form of plaque, but also associated with gingivitis, periodontitis, and periapical disease . Oral flora, particularly streptococcus, are implicated in approximately 40 percent of cases of infective endocarditis . Bacteremia can arise from dental procedures but also from such routine activities as tooth brushing and chewing, especially if extensive oral infection and inflammation are present. Patients with dentures may develop bacteremias associated with poorly fitting dentures.

Although dental procedures for hygiene, restoration, or extraction of diseased teeth can cause breaks in the epithelium lining the mouth that allow bacteria to spill into the bloodstream causing bacteremia, most cases of orally related endocarditis are not attributed to dental procedures . The burden of oral disease of itself is a concern for the patient at high risk of endocarditis .
