If you dont sign up for Part B when youre first eligible, you may be required to pay a late enrollment penalty when you do choose to enroll. Additionally, youll need to wait until the general enrollment period .

With the late enrollment penalty, your monthly premium may go up 10 percent of the standard premium for each 12-month period that you were eligible but didnt enroll. Youll continue to pay this penalty for as long as youre enrolled in Part B.

For example, lets say that you waited 2 years to enroll in Part B. In this case, youd pay your monthly premium plus 20 percent of the standard premium.

A Brief History Of The Medicare Part B Deductible

Way back in 1966 when Medicare first showed up on the radar, the Medicare Part B deductible came in at $50. It hung out around there for seven years or so before it saw an increase to $60 in 1973. Then, it stayed at $60 for another 9 years until 1982 when it got another bump to $75. It hung out there until 1991 when it got a $25 bounce up to $100.

From there, nobody touched the Medicare Part B deductible for 14 years. It held at $100 from 1991 until 2004. Take that inflation.

After that, we started seeing this number move around pretty regularly. I summarized those amounts below in a nice easy to understand table. It looks like it was held flat during the recession years at $135. Things crept up from there but were reset in 2012 back down to $140 which is the only year we ever saw a reduction. Max predicts we are set to break $200 in 2021, and Ill wager $200 on that if there are any takers out there.

I suppose we shouldnt complain too much. According to this random inflation calculator from the internet, $50 in 1966 would be worth about $406 in 2020. So we have it easy compared to our 1966 counterparts.

The Part B deductible has generally increased over time, although there have been some years when it stayed the same or even decreased. The increase for 2022 was the largest year-over-year dollar increase in the programs history. But the decrease for 2023 was the first time the deductible has declined in over a decade. Higher Premiums For Enrollees With High

Since 2007, people who earn more than $85,000 have paid higher Part B premiums based on their income.

For the first time, the threshold for what counts as high income was adjusted for inflation as of 2020, increasing it to $87,000 for a single individual and $174,000 for a couple. It increased again for 2021, 2022, and 2023. Harry Sit, of The Finance Buff, explains how the inflation indexing works here.

Indexing the high-income threshold: The mathThe indexing is based on the percentage by which the average of the Consumer Price Index for Urban consumers for the 12-month period ending in the most recent August exceeds the average of the 12-month period that preceded that. So for 2021, for example, we can look at how the average CPI-U from September 2019-August 2020 exceeded the average CPI-I from September 2018-August 2019.

On this page, you can pull up the data for CPI-U and manually calculate how the average CPI-U has changed. Youll add up all the numbers from September 2019 through August 2020 , and divide by 12 to get the average . Then youll do the same thing for September 2018 through August 2019 . The difference between those two numbers is 3.705, which represents a 1.46% increase from the 254.016 average CPI-U for September 2018 to August 2019.

So as Sit explains here , we increase 87,000 by 1.46% which results in 88,270 and then round to the nearest $1,000. That gives us an income threshold of $88,000, which was the lower bound of high-income as of 2021.

For 2021 the Medicare Part A premium for most people will be $0. If you have to buy part A then the premium will be up to $471 per month, compared to $458 in 2020.

The Part A hospital deductible is $1484 which is up from $1408 in 2020.

The Part A deductible applies to each benefit period. This benefit period starts when you enter the hospital and ends when you are out of the hospital for 60 days in a row.

The Part A Coinsurance for days 61 90 is $371 per day which is up from $352 in 2020. Coinsurance for days 91 and beyond is $742 per day up from $704 in 2020.

In November 2021, CMS announced that the Part B standard monthly premium increased from $148.50 in 2021 to $170.10 in 2022. This increase was driven in part by the statutory requirement to prepare for potential expenses, such as spending trends driven by COVID-19 and uncertain pricing and utilization of Aduhelm.
