While Extra Help can assist in paying for Part D costs, there are other programs you may be eligible for that help with Part B costs.

Part B is outpatient medical coverage.

Your Part B monthly premium is income-based.

Depending on your income, there are different programs you may qualify for.

We talked about Part B in more detail in another post, which you can read here.

There are 3 Medicare Savings Programs that can help with Part B costs:

  • Specified Low-Income Medicare Beneficiary Program
  • Qualified Medicare Beneficiary Program

We will talk about each of these briefly below.

Medicare Helps Low Income People

Of the 54 million people with Medicare, a staggering 25% have annual incomes below $14,400. For these people living in retirement, or coping with a disability in poverty, Medicare coverage offers a lifeline, a chance to get needed health care. That precious red white and blue Medicare card means that a doctor will see you, a physical therapist will help you recover from a stroke and much more. But that lifeline disappears if you cant afford to pay your $105 monthly Medicare Part B premium, or your $1260 hospital deductible, or if you have 10 prescriptions that need filling, each with a co-pay of $20 or $30.

Dual eligibles, people who qualify for both Medicare and Medicaid, need these safety net programs to remain strong and to grow. When states seek instead to narrow Medicaid eligibility standards, they not only are cutting off people from needed Medicaid services, like long-term services and supports, but also are effectively making it impossible for dual eligibles to use their Medicare. Medicare is a benefit that many earned working their whole lives at low-paying difficult jobs. Like other workers, they saw their Medicare contributions deducted from each and every paycheck. We at Justice in Aging are committed to working with the Center for Medicare Advocacy and other advocates to ensure that all Medicare beneficiaries, especially those who most need what Medicare can provide, can afford to use the benefits to which they are entitled.

Canceling Part B Because You Got A Job With Insurance

If you have had Part B for a while but no longer need it because youve rejoined the workforce with access to employer-sponsored health insurance, congratulations! But before you drop Part B, find out if your jobs coverage is primary or secondary to Medicare.

A primary payer health plan pays before Medicare. That means your employer-provided health plan will cover its share of your health care costs first, and if theres anything left over that Medicare covers, Medicare will pay what remains.

Conversely, a secondary payer health plan covers only costs left over after Medicare covers its share.

If your health plan at work is a primary payer, thats great. Feel free to drop your Part B coverage if you wish. The Part B premiums might not be worth any additional coverage you receive. But if you have secondary-payer insurance at work, its usually better to keep Part B, or you could get stuck paying Medicares share of your health care expenses.

Talk to your human resources department at work to find out if your employer-sponsored plan is primary or secondary to Medicare. Generally, businesses with 20 or fewer employees have secondary payer plans, while larger companies have primary payer plans.

How To Appeal A Part B Premium Income Adjustment

You may request an appeal if you disagree with a decision regarding your income-related monthly adjustment amount. Complete a Request for Reconsideration or contact your local Social Security office to file an appeal.

You may be able to skip the formal appeal and simply provide documentation if your income changed due to any of the following:

  • You married, divorced or became widowed.
  • You or your spouse stopped working or reduced your work hours.
  • You or your spouse lost income-producing property due to a disaster or other event beyond your control.
  • You or your spouse experienced a scheduled cessation, termination or reorganization of an employers pension plan.
  • You or your spouse received a settlement from an employer or former employer because of the employers closure, bankruptcy or reorganization.

These methods apply to the Part B premium. Contact the IRS if you disagree with your adjusted gross income amount, which is provided to Medicare by the IRS.

Do You Have To Apply For An Msp During Medicare’s Annual Election Period

No. You can apply for MSP assistance anytime. As noted above, youll do this through your states Medicaid office, which accepts applications year-round.

But the marketing and outreach before and during Medicares annual election period can be a good reminder to seek help if you need it. You might decide to make a change to your coverage during the annual open enrollment period, and simultaneously check with your states Medicaid office to see if you might be eligible for an MSP or Extra Help with your drug coverage.

What Is The Medicare Part B Giveback Benefit

The Medicare Giveback Benefit is a Part B premium reduction offered by some Medicare Part C plans.

If you enroll in a Medicare Advantage plan with this benefit, the plan carrier will pay some or all of your Part B monthly premium. The amount covered can range from 10 cents to the full Part B premium cost .

The Consolidated Omnibus Reconciliation Act allows most employees and their family members to continue coverage after their employment ends usually for up to 18 months. But having COBRA benefits does not mean you can safely delay signing up for Part B. Individuals who delay enrolling in Part B because they have COBRA coverage will not receive a SEP to enroll in Part B later.

Furthermore, COBRA carriers may recoup what they paid toward your medical bills when they discover you were eligible for Medicare but not enrolled in it. This is because COBRA plans cover only the portion of your health care claims Medicare wouldnt be responsible for paying even if you dont have Medicare.

COBRA insurers may not know youre eligible for Medicare at first. But by the time your COBRA plan ends, the insurer usually becomes aware of your Medicare eligibility and may begin recouping that Medicare should have paid on first. This usually occurs after the Part B SEP has ended, causing you to wait to enroll in Part B during the general enrollment period from January to March of each year.

Enrolling in Part B during the GEP means Medicare coverage wont begin until July of the year you enroll .

    • Check to see if your state participates in a State Pharmaceutical Assistance Program , which can help you save money off the direct cost of your medications. Not every state participates in fact, fewer than half do. But this is a good option if you live in a state that does.
