Medicare To Cover Medically Necessary Conditions Requiring Dental Services
Beneficiaries of Medicare will now be able to receive dental services if they meet certain medical requirements. January 1, 2023, will mark the start of expanded dental coverage
The final rule covers the following instances:
Reconstruction of a ridge when performed as a result of and at the same time as the surgical removal of a tumor. Stabilization or immobilization of teeth when done in connection with the reduction of a jaw fracture. Extraction of teeth to prepare the jaw for radiation treatment of neoplastic disease. Dental splints only when used in conjunction with medically necessary treatment of a medical condition. Dental services including both examination and treatment prior to cardiac valve replacement, valvuloplasty or organ transplant procedures.
The final rule does not expand Medicare to cover dental services for patients with conditions such as diabetes.
How Current Proposals Would Add A Dental Benefit To Medicare
In the 116th Congress, the House of Representatives passed the Elijah E. Cummings Lower Drug Costs Now Act , which among many provisions, provided for dental coverage under Part B of the Medicare program, starting in 2025 if enacted into law. Medicare would cover 80% of the cost of preventive services and basic treatments, and would phase up coverage for more major treatments, covering 10% of the cost in 2025, increasing 10 percentage points per year to 50% in 2029 and thereafter. The Congressional Budget Office estimated this benefit would cost almost $238 billion over 10 years . The recently reintroduced version of H.R. 3 does not include coverage for a dental benefit.
Representative Doggett introduced the Medicare Dental, Vision, and Hearing Benefit Act in July 2021 that would provide dental coverage under Medicare Part B. In contrast to H.R.3, the proposal, according to a press release, would require no coinsurance for preventive services, and impose a 20% coinsurance for more extensive benefits, which is similar to how the Part B benefit is structured for physician and other outpatient services, and is lower than the 50% coinsurance typically charged by Medicare Advantage plans.
How To Enroll In Dental Coverage With Medicare
If you wish to enroll in dental coverage with Medicare, you have a couple of options. You may select a Medicare Advantage plan that includes dental coverage, or you may enroll in a stand-alone dental plan. If you choose a Medicare Advantage plan, you cannot just sign up for it at any time. If you are not already enrolled in one of these plans, then you will need to wait until the open enrollment period to sign up. Similar to group health insurance through your employer, the ability to sign up for coverage is limited to certain times of the year. During this period, you can also switch from Medigap coverage to a Medicare Advantage plan.
If you decide to sign up for a stand-alone dental policy and keep Original Medicare, you should contact the plan administrator to determine how to enroll. Many of these plans also have open enrollment periods during which you can. A local insurance agent can help you find plans that will work for you, and they can also help you with getting signed up. Once you enroll in coverage, you should receive an insurance card that should be used whenever you receive dental treatments.

Dental Services For Medicaid Beneficiaries At Two Local Nonprofits
Seniors on Medicaid in Washington state know how difficult it is to find a dentist to care for them. In April, the Seattle Times shined a light on the issue when it exposed a budget deficit at the University of Washingtons School of Dentistry caused in large part because 65 percent of the patients the school sees are on Medicaid, which doesnt cover the cost of the services.
But one dentist and two Seattle nonprofit organizations are bucking the trend by providing dental care for seniors on Medicaid. The program involves Dr. Allison Jacobs LeGendre, a veteran dentist who works with hundreds of these seniors at Foss Home and Village and at Providence ElderPlace, both in Seattle.
LeGendre, who is affiliated with the UW and has had a long career as a dentist and dental products designer, is trying to change the way seniors receive dental care. As a Foss dental services employee, she is able to provide comprehensive dental carecleanings, extractions, fillings and denturesto meet the needs of a population that otherwise has a very hard time finding dental care.
Few dentists serve Medicaid patients because the reimbursements are so low. And those who do almost always cap the number of Medicaid patients theyll see. Luckily for the residents at Foss and the participants at ElderPlace, they have a place to go.

A secondary service that is related to the teeth or structures directly supporting the teeth unless it is incident to and an integral part of a covered primary service that is necessary to treat a non-dental condition and it is performed at the same time as the covered primary service and by the same physician/dentist. In those cases in which these requirements are met and the secondary services are covered, Medicare does not make payment for the cost of dental appliances, such as dentures, even though the covered service resulted in the need for the teeth to be replaced, the cost of preparing the mouth for dentures, or the cost of directly repairing teeth or structures directly supporting teeth .
