Are There Exceptions To The Mbi Requirement
MBIs are required, with three exceptions:
- Medicare appeals: When you file an appeal, you can choose whether to use an MBI or an HICN.
- Checking the status of a claim: If you filed a claim before January 1, 2020, you can use an HICN to check the status of a claim. If you filed it after that date, youll have to use the MBI.

Hard Facts About Medicare
Here are some important things for seniors to know about how Medicare and the Social Security Administration function.
- Medicare generally wont call you, except in limited circumstances. One such exception is if you have called 1-800-MEDICARE and requested a return call.
- Medicare will never call or come to your home uninvited to sell products or services.
- SSA representatives may call Medicare beneficiaries if they need more information to process applications for Social Security benefits or enrollment in certain Medicare Plans, but, again, this is rare.
- If a phone call is needed, youll receive an official letter from the SSA to arrange a telephone interview.
- Medicare cards do not expire, so be wary of someone saying they need to send you a new one.
- If your card is lost or destroyed, contact the SSA directly to request a replacement. If you think someone else is fraudulently using your Medicare card, then call Medicare directly at 1-800-MEDICARE .

How To Find A Medicare Number
The Social Security Act of 1964 established the Social Security Administration which oversees Medicare, a health provider for individuals age 65 and older or under age 65 with certain disabilities. Locating a Medicare number is necessary for obtaining a replacement card and verifying benefits.
Look at your social security card. Your social security number is the first part of your Medicare number for part A and B benefits. The second part is the letter A or B, depending on which benefit you are needing the number for. Part A is inpatient hospital benefits and Part B is outpatient medical benefits. For example, if your social security number is 111-22-3333, then your Medicare number for Part A benefits is 111-22-3333-A. If you do not have a social security card or your Medicare card, contact your local SSA office for a list of documents required for obtaining a replacement card.
Contact your Medicare Advantage plan provider, if you have a Medicare Advantage plan. Provide the plan provider with your social security number, full name and date of birth, then the provider can provide you with your Medicare Advantage plan number. Medicare Advantage plans act as a supplement to Part A and B coverage.
Contact your Medicare Part D prescription drug coverage provider. This number is located on the provider’s website and on many documents from the provider. You will need your date of birth, Social Security number and full name for obtaining your number.

If you have not yet enrolled yourself and/or your family, there are multiple methods you can use to enroll in Medicaid. Your first option is to enroll yourself and/or your family by going online and filling out an application for yourself and/or your family at the Health Insurance Marketplace. There you can follow the steps to create an account and provide your information such as whether or not you have a disability, your income level, and enroll any minor dependent children you have in health coverage programs such as the Childrens Health Insurance Program, otherwise known as CHIP.
Another method you can use to enroll in the Medicaid program is to directly contact your states Medicaid agency. You can find the agency contact information you need by visiting the Healthcare website. Scroll down to the state in which you reside, and you will be taken to the website of your local Medicaid agency. There you will be provided with telephone numbers for your local Medicaid agency which you can call during normal business hours in order to check if you and/or your family qualify for Medicaid.

How Do You Look Up Your Medicare Number
Medicare numbers can be found on Medicare cards and also on much of the correspondence from the Social Security Administration or the Medicare provider. Medicare numbers can also be obtained by visiting a local SSA office or by calling the Medicare provider and establishing the members identity.
A Medicare number includes a persons social security number followed by a small code that consists of letters or letters with numbers depending on various factors. It is also called a Medicare claim number on documents from the provider. Because the number is primarily made up of a persons social security number, it is not easy to look it up if lost or forgotten.
How Do I Find My Medicare Number
If youre asking yourself how to look up my Medicare number, what is my Medicare number? or where can I find my Medicare number? We can help! Once you become eligible for Medicare and enroll during the general enrollment period , you will receive a Medicare number that will become active beginning on your effective date.
After you enroll, looking up your Medicare number is actually quite simple. Your Medicare identification number is located on your red, white, and blue Medicare card. The standard Medicare number format contains eleven random letters and numbers, each of which are arranged in a specific pattern that is used to identify you and only you. Formerly, the Medicare number consisted of a persons Social Security Number along with an identifying letter at the end. These days however, new regulations have replaced this original Medicare number format. Your Medicare number is now also known as your Medicare Beneficiary Identifier, or MBI, and its specific sequence of random numbers and letters are in place in order to better protect the identities of Medicare beneficiaries.
Additionally, paperwork and documentation from the Social Security Administration and Medicare will also show your Medicare Beneficiary Identifier.
Otherwise, you should be able to go to your local Social Security Administration or Medicare office. Alternatively, call Medicare and properly establish your identity, then request a new Medicare card.

Applying For Your National Provider Identifier
Whether youre just beginning your physical therapy career or are a seasoned veteran, having your NPI number should be a priority.
This 10-digit identifier is the number that must be used on claim forms submitted to payers by individual and organization health care providers who meet a broad definition of a covered entity under the Health Insurance Portability and Accountability Act, or HIPAA. An NPI also is required for all providers enrolled in Medicare. The NPI replaces any legacy or billing numbers for all health insurance plans, both public and private.
Who Can Get an NPI
All health care providers who are HIPAA-covered entities, whether individuals or organizations, must obtain an NPI. A covered entity is defined as a health care provider that transmits any health information in electronic form, such as submitting a claim. This includes:
- Health care providers that conduct certain transactions in electronic form.
- A health care clearinghouse.
- A health plan .
If you electronically transmit health information in connection with a HIPAA standard transaction, you are considered a covered entity even if you use a business associate to do so.
There are two categories of health care providers for NPI purposes: Type 1 and Type 2 .
Group practices, in addition to the individual physical therapists working in the practices, are each required to obtain an NPI.
