What Is A Medicare Claim

Medicare claims ask Medicare or your insurer to pay for medical services or care you have received. In almost all cases, your doctor or a hospital where you received care will send the claim directly to Medicare if you are covered by Medicare Part A or Part B.

If you have a Medicare Advantage plan, there is no claim. The private companies that administer these plans are under contract with Medicare and are paid a set amount each month.

For Part D prescription drug plans, the insurance company that administers the plan contracts with pharmacies. Both those preferred pharmacies as well as nonpreferred pharmacies not in your plan will bill the insurer. You may have to pay more out-of-pocket if you use a nonpreferred pharmacy.

As you’re using your health plan, you may wonder how and why you may need to submit a claim. If you get your health insurance through your employer, here are some tips to help guide you.

In most cases, when you go to a network provider, you will not need to submit a claim for your care. However, there are a few occasions where submitting a claim on your own may be required, in order for you or the provider to receive reimbursement. Some examples include:1

How Do I Read The Medicare Summary Notice

Its really two notices. Youll receive a separate MSN for your claims for Part A which helps pay for inpatient hospital care, inpatient care in a skilled nursing facility, certain home health care services and end-of-life hospice care and your claims for Part B .

The Part A Medicare summary notice shows the dates of the claims, how much of the deductible you have met, whether Medicare approved all of the claims, and the total amount you may be billed from the facility. It also lists the facilities where you have had claims this period and the dates you received services there.

Page 2 includes tips for reviewing the notice and how many days of coverage you have used in your benefit period for inpatient hospitalization and skilled nursing facilities.

Page 3 provides detailed information about your claim, including the number of benefit days used, whether the claim was approved, any charges that werent covered, the amount Medicare paid and the maximum you may be billed. Keep in mind that the bill may be covered by Medigap or other supplemental coverage.

The âBlue Buttonâ within the system is the tool you use in order to download claim information. In other words, look for the Blue Button when youâre in your profile so that you can download and save your personal health information file.

You can save it to your computer, print it out, and share and email it with others. Medicare says its Blue Button is âsafe, secure, reliable, and easy to use.â

The Blue Button 2.0 actually lets you connect your Medicare health information to other services like apps and research programs. This version of the Blue Button is called an API, and it contains four years of Medicare Part A, B, and D data for over 53 million Medicare beneficiaries.

How To Read An Eob

EOBs may look different depending on your provider, but they generally include similar information. An EOB summarizes the items and services you accessed, the amount charged to Medicare, how much was covered by Medicare and how much you owe.

Below is a sample EOB with different sections labeled and explained.

After reviewing your EOB, if you have any questions or complaints, call your health insurance provider.

How To File A Medicare Claim Yourself

If you need to file your own Medicare claim, youll need to fill out a Patient Request for Medical Payment Form, the 1490S. Make sure its filed no later than 1 full calendar year after the date of service. Medicare cant pay its share if the submission doesnt happen within 12 months. You can log in to and view your claims to ensure they are being filed in a timely fashion. If your claims arent being taken care of, contact the doctor and ask them to file the claim.

    MSNs are released quarterly, but you can request monthly electronic MSNs for faster tracking.

    Checking If A Claim Has Been Accepted Or Rejected By Medicare Australia

    All claims transmitted using online claiming have up to three stages: Sent, Processed, Paid. After a claim is sent it generally takes 1 day to be processed, but in some cases, if there are problems it can take a week or longer to be processed.

  • On the File> Online Claiming> Bulk Bill Claims tab, in the encounters list, select the claim you want to investigate.
  • If the claim has not been processed, the status remains unchanged.
  • If the claim has been processed, it changes the state to one of the following states depending on the result of the processing report returned by Medicare Australia:
  • Claim paid by Medicare Australia – no further action is required
