Medicare can work well in conjunction with some FEHB plans. Here is some important information to consider when selecting benefits for the coming year.
A few days back, I provided some general advice on how to make Open Season choices. A lot of the comments focused on Medicare questions, so here is a quick follow-up on that topic.
Readers have only a few days to make Medicare Part B choices and not many more days to make FEHB choices , so a laser-like focus is needed.
As I explained in my previous column, Washington Consumers CHECKBOOK staff read the brochures for over 250 FEHB plans, and enter premium, coverage, and cost information into elaborate computer programs that calculate likely and not so likely cost under each plan. These programs use actuarial data on the risk of health care costs of varying amounts based on age, family size, health status, and other factors. They produce estimates of health care costsenrollee share of premium plus out of pocket costsfor a good, bad, or average year if a family like yours were enrolled in each of those plans.

Medicare Has Four Parts:
- Part D . There is a monthly premium for Part D coverage. Most Federal employees do not need to enroll in the Medicare drug program, since all Federal Employees Health Benefits Program plans will have prescription drug benefits that are at least equal to the standard Medicare prescription drug coverage. Still, you may want to be aware of the benefits Medicare is offering, so you can help others make informed decisions. If you have limited savings and a low income, you may be eligible for Medicare’s Low-Income Benefits. For people with limited income and resources, extra help in paying for a Medicare prescription drug plan is available. Why is FEHB so expensive?
There are a number of reasons why having FEHB is so rare and valuable. First, few employers offer comparable coverage. This means that if you pay $400 per month in FEHB premiums, the total premium cost will be approximately $1,400 per month. It would cost a lot more than that to find comparable coverage on your own.

Why Should I Consider Enrolling In Medicare
When you turn 65 and are retired, many health insurance carriers assume that youll be on Medicare Part B. In turn, many people experience slight changes in their health insurance coverage as time goes on, even with FEHB. Does it impact everyone? Sometimes not someones health needs may be more simplistic and their FEHB in retirement sufficiently covers their needs. For others, it might be different.
Remember, health insurance carriers are for-profit organizations, even those through FEHB. As such, they will make business decisions about certain health coverages in a manner that works best for them. Carriers frequently change their health care coverage, and some day that coverage change could impact you in a way that can be devastating. This is why we always encourage the families we serve to review their health coverages again in open season, even if theyre not making changes.
I have a client who personally experienced receiving health services that were no longer covered by FEHB as they progressed through retirement, because the insurance carrier passed on that coverage responsibility to Medicare as the primary insurer. They didnt have Medicare Part B, which in turn required them to pay out of pocket for those expenses.

Can I Keep My Fehb After Age 65
Your FEHB coverage will continue whether or not you enroll in Medicare. If you can get paid Part A coverage, we recommend that you sign up for it. Most federal employees and annuitants are entitled to Medicare Part A at age 65 at no cost. If you dont enroll in Medicare, your FEHB plan pays for benefits in full.

Many readers have contacted me with questions about Medicare, which has strict coverage rules and payment provisions that can sometimes be confusing. Here are responses to two recent inquiries.
Q. I am a federal retiree. I pay almost $400 a month for coverage through the Federal Employees Health Benefits program. It would save us a lot of money if we bought a Medigap plan to supplement our Medicare coverage instead. Please tell me the pros and cons.
A. Like retired workers at some private companies, many federal retirees can keep their health-care coverage to supplement Medicare once they become eligible for Medicare at age 65.
If you enroll in Medicare Part A and Part B , that Medicare coverage becomes your primary insurance, and your federal retiree plan acts as secondary coverage for services or costs not picked up by Medicare.
With Medicare as your primary insurer, your current FEHB plan retiree plans are the same as those offered to current workers may provide more coverage than you need.
If your FEHB premium is a stretch financially, see if theres a lower-cost FEHB plan that would work more affordably as secondary coverage with Medicare, says David Lipschutz, a policy attorney with the Center for Medicare Advocacy.

Do I Need To Sign Up For Medicare Part B
- Medicare Part B may provide coverage for things that FEHB doesnt cover or has limited coverage for
- Medicare Part B also allows you to access any health care provider that accepts Medicare, while FEHB only provides access to a limited number of in-network providers.
- If you do not enroll in Medicare Part B during your initial enrollment period, you must wait for the general enrollment period to enroll, and Part B coverage will begin the following July 1 of that year.
- If you wait 12 months or more after first becoming eligible, your Part B premium will go up 10 percent for every 12 months that you could have had Part B but didnt take it. You will pay the extra 10 percent for as long as you have Part B unless your current coverage is considered creditable.
Is There A Late Enrollment Penalty
If an individual did not sign up for Part B when he or she was first eligible, then the Part B monthly premium may be higher. In particular, the cost of Part B may go up 10 percent for each 12-month period that an individual could have been enrolled in Part B but did not sign up for it. The individual will have to pay this extra monthly premium as long as the individual has Part B, except in special cases. For those individuals who miss the deadline to enroll in Medicare Parts A and B when they are first eligible, Medicare has an open season enrollment for Parts A and B each year between January 1 and March 31 with coverage becoming effective the following July 1. But a late enrollment penalty for Part B will apply equal to 10 percent per year of the first tier Part B monthly premium amount in effect that year . The penalty period is measured from the last month the individual could have enrolled in Medicare and July 1 of the year Medicare Part B becomes effective.

Is Fehb Better Than Medicare Part B
Federal retirees with HMO plans dont need to join Part B. HMOs generally cover most medical services with small copayments at the time of each service.
The cost of seeing doctors outside of your HMO plan network can be financially taxing. For these retirees, joining Part B may be the better option because it covers costs for out-of-network services.
Traveling within the country is common for many, especially during retiring years. Individuals that travel may consider purchasing non-emergency coverage while out of town.
Part B will cover these costs, whereas FEHB generally includes emergency care when traveling outside the U.S. plus dental and vision benefits.
For some, paying the Part B premium is worth the benefits.
Should You Enroll In Medicare Part B
Part B is more complicated, mainly because everyone must pay a premium for it.
There are differences between what Medicare and an FEHB plan cover. According to the U.S. Office of Personnel Management , some FEHB plans may cover certain items that Medicare doesnt cover, including but not limited to:
- Routine physicals
- Emergency care outside of the United States
- Some preventive services
- Dental and vision care
Medicare may cover some services and supplies that some FEHB plans may not cover, including but not limited to:
- Some orthopedic and prosthetic devices
- Durable medical equipment
- Home health care
- Limited chiropractic supplies
Part B also may help if you are in an FEHB HMO plan. These plans have a limited provider network. Part B coverage would allow you to use a Medicare-participating provider outside the HMO network.
Its important to note that if you dont enroll in Medicare Part B when you first become eligible, you may be required to pay a Part B late enrollment penalty if you decide to enroll in the future. Your Part B premium would increase 10% for every 12-month period you could have had Part B coverage but didnt take it, and you would pay this additional fee for as long as you have Part B.

Can I Stop My Medicare Part B
You can voluntarily terminate your Medicare Part B . Its a serious decision. You must have a face-to-face meeting with Social Security before you can terminate your Medicare Part B coverage. Call the SSA or your local Social Security office to schedule your interview.
What happens if you opt out of Medicare Part B?

Special Exception For Tricare Recipients
The final note that I do want to make is for any of our readers who are under the military health program called TRICARE. For those of you under TRICARE, you will be required to enroll in Medicare Parts A and B when you turn age 65. Its simply part of the requirement to retain TRICARE at 65 . I know I said it was a choice for everybody else earlier in this article, but if a TRICARE recipient fails to enroll in Medicare Part B at age 65, they will permanently lose TRICARE COVERAGE. Hopefully, that helps clarify things for this special audience.
