Effectiveness Of Dental Care In Improving Health Outcomes For Head And Neck Cancer Patients Treated With Radiation
No randomized controlled trials have compared the effectiveness of tooth-preserving protocols and protocols that emphasize tooth extraction without preventive care for head and neck cancer patients undergoing radiation therapy. One of the earliest retrospective studies comparing tooth-preserving and aggressive tooth extraction protocols was carried out at Walter Reed Army Medical Center . It showed that patients in the tooth-preserving protocol lost fewer teeth than their predecessors treated under an aggressive tooth extraction protocol. Patients in the tooth-preserving protocol also required fewer dental visits both before and after radiation therapy . ORN rates were historically low before and after the change in protocol, so the study did not demonstrate an effect on ORN.
Studies to identify causes of ORN have repeatedly identified extractions and lack of preventive dental care as major contributing factors . As tooth-preserving treatments have become more common and the rate of ORN has dropped, unknown causes of ORN are proportionately more likely to be identified as causes than are extractions.
What Happens During A Root Canal
The American Association of Endodontists describes the process like this:
- The dentist numbs the affected tooth and places a dental dam around it to keep it clean and dry during the procedure.
- She then drills a small hole in the tooth, inserts special instruments into the hole to clean the infection from the canals, and shapes the tooth for filling and restoration.
- The canals are filled with a rubbery material called gutta-percha and then sealed.
- The dentist then places a temporary filling to close the hole.
- A crown or other restoration is placed to restore the tooth and return it to full function.
What Dental Care Does Medicare Part A Cover
Medicare Part A won’t cover dental services such as cleaning, fillings, root canals, crowns, extractions or dentures.
However, if you have an emergency or complicated dental procedure where you need to go to the hospital, Medicare Part A can cover your hospital care. This could include jaw surgery, tooth extractions before cancer radiation treatments, splints for facial fractures, an oral exam before a kidney transplant or other major treatments.

Recipients For At Least 24 Consecutive Months
If you have been a recipient for 24 or more consecutive months, in addition to the dental services for which you qualified when you started receiving benefits, you are eligible for the following services related to acrylic dental prostheses :
- Manufacture of a complete or partial acrylic prosthesis and replacement after 8 years
- Re-coating every 5 years, but after a minimum of 3 months after obtaining the prosthesis
- Replacement of lost or damaged prostheses , covered at 50%
- Replacement of prostheses following oral surgery
- Repair of prostheses
- Addition of a structure to an existing partial prosthesis
To receive these services, you must obtain prior written authorization from your local employment centre and present it to your dentist or denturist.

Services Excluded Under Part B
The following two categories of services are excluded from coverage:
A primary service provided for the care, treatment, removal, or replacement of teeth or structures directly supporting teeth, e.g., preparation of the mouth for dentures, removal of diseased teeth in an infected jaw.
A secondary service that is related to the teeth or structures directly supporting the teeth unless it is incident to and an integral part of a covered primary service that is necessary to treat a non-dental condition and it is performed at the same time as the covered primary service and by the same physician/dentist. In those cases in which these requirements are met and the secondary services are covered, Medicare does not make payment for the cost of dental appliances, such as dentures, even though the covered service resulted in the need for the teeth to be replaced, the cost of preparing the mouth for dentures, or the cost of directly repairing teeth or structures directly supporting teeth .

Coverage And Claims Inside New Brunswick
New Brunswick Medicare pays for the following insured practitioner services:
- most medically required services provided by a physician in either a physicianâs office or in an approved hospital and
