What Are The Options For Medicare Extension

There are several options that could be considered for extending Medicare coverage to Mexico . Here we describe four policy options and the issues that would need to be addressed in order to purse each.

Option 1: Expand Original Medicare to Allow Reimbursement of Mexican Providers

The Original Medicare fee-for-service program consists of Part A and Part B, which cover inpatient and physician services, respectively. Medicare-eligible participating providers are reimbursed directly for their services, with reimbursement rates determined by a complex set of rules. In theory, the administrative infrastructure could be expanded to allow Mexican providers to be reimbursed through Parts A and B, thereby expanding Medicare coverage to Mexico. Because of the complex rules governing reimbursements, and the differences in the health economies of Mexico and the United States, such an expansion would require a substantial investment in administrative infrastructure to address the determination of Medicare-eligible provider status, reimbursement rates, means for coordination with existing Mexican institutions, and premium rates.

Additional Insurance For International Travel

If youre on a budget, another option is to obtain supplemental travelers insurance. This isnt medical insurance, but is instead a short-term plan that covers emergencies while youre out of the country. You may also be able to buy short-term insurance through a travel planner.

The catch is that youll need to buy the coverage ahead of time for a specified itinerary. You cant buy travelers insurance once youve already left the country.

Also, not all supplemental plans cover preexisting conditions. If you have chronic health conditions, be sure to review the exclusions before you purchase travel insurance.

In Mexico, access to public health care is cheap. The main downfall is that as most public healthcare systems, the demand is higher than the offer, so the system is overcrowded. For this reason, many expats choose to either get private insurance or supplemental coverage.

Mexico has various trusted providers. Costs Can Change If You Are Admitted To The Hospital

If an ER visit results in being you admitted to the hospital, then the visit is considered part of an inpatient stay and ER-related copays would not apply. To qualify as such, a hospital admission must happen within three days of the ER visit for the same or a related condition, and it must be at the hospital where ER services were provided. Admission to a different hospital within three days, even for the same condition, would be considered a separate event.

When applying to be enrolled under the public health insurance scheme in Mexico, you need the following documents:

  • A valid Identification Document
  • A certified copy of your Marriage Certificate
  • A certified copy of Birth Certificate/s
  • Your Unique Population Registry Code
  • Proof of payment of the fee for each member of the family group
  • Proof of address
  • The medical questionnaire which is provided by the IMSS

Do Medicare Supplement Policies Cover Foreign Travel

If you buy a private Medicare supplement policy, better known as Medigap, you may have some coverage for foreign travel emergencies. Medigap Plans C, D, F, G, M and N cover emergency health care while you travel outside of the United States. Plans C and F are no longer available to new Medicare beneficiaries. Only people eligible for Medicare before 2020 can enroll in those two plans.

These Medigap plans cover foreign travel emergency care that begins in the first 60 days of your trip. They pay 80 percent of the billed charges for certain medically necessary emergency care outside the United States, but first you pay a $250 deductible for the year.     If you regularly travel to the US, you should choose a policy with worldwide coverage, including the USA. However, this will bring up the price of your premium, considering the high cost of healthcare, but it is better than facing those prices without health insurance.

    If you do not intend to go back to the USA, then you can choose a policy that does not include the US, thus pay lower monthly/annual premiums.

    Just as with trips to a foreign country, Medicare will not cover your medical expenses that occur during a cruise. But there is a very small window, starting from when the cruise starts, during which you have Medicare protection:

    • The present doctor is authorized to provide medical care to you
    • The ship is still in US waters, not international waters, and
    • The ship is within 6 hours away from a US port, regardless of whether it is an emergency or not. If you are more than six hours away from the port, then Medicare will not cover your medical expenses.
