Both Medigap and Medicare Advantage plans typically only cover healthcare services that are already covered by Original Medicare.

Some companies do, however, offer additional coverages that Original Medicare does not. For example, many Medicare Advantage Plans offer dental, vision, and hearing coverage which is not covered by Original Medicare unless it is a health emergency,

Although the Wellness visit covered by Original Medicare, Medigap, and Medicare Advantage is somewhat less comprehensive than a traditional annual physical, it is still an important benefit that every enrollee should take advantage of.

If diagnostic testing is ordered, it may or may not be covered depending on the insurance company you select.

Cpt Codes For Annual Wellness Visits

Below, we break down the four types of billing codes for AWVs, as well as advanced care planning.

As mentioned above, AWVs take three forms. The programs CPT billing codes reflect each. And while the three varieties possess specific distinctions or requirements, the key difference is when a certain AWV can be given and billed for.

For an IPPE, your patients may only receive this benefit within the first 12 months of their Medicare enrollment. It is considered a once in a lifetime assessment. After the initial eligibility period, your patient cannot receive an IPPE. It is also dependent on a health risk assessment .

An IAWV is practically identical to an IPPE, except it is available to your patient after 11 months of Medicare enrollment. It is for patients that miss their window for an IPPE. An HRA drives the IAWV process, as well. But this screening also includes an optional cognitive exam and end of life planning.

The Subsequent AWV is the yearly follow-up to an IAWV. Eleven months after an IAWV, a patient can attend these sessions to modify and maintain their preventive care plan, based on how their health is at any given time.

As the years pass, and the patients health evolves, the doctor may use the Subsequent AWV to guide individuals toward other Medicare preventive programs, such as Chronic Care Management , Behavioral Health Integration , or Remote Patient Monitoring .

How Can An Annual Wellness Visit Produce Revenue

As for AWV reimbursement rates, what is the revenue opportunity of the program?

What if 500 of your patients receive an annual Subsequent AWV, while 100 of these patients also participate in CCM every month?

Offering an AWV program can generate revenue, as well as broaden care access for your patient. The combination is inherently valuable.

Medicare Annual Wellness Visit Vs Annual Physical Exam

CMS notes that a “routine physical examination” is not covered by Medicare. As such, Medicare patients will be expected to cover the entire cost of the service .

Unlike a yearly physical exam that can take about 30 minutes during which a physician measures all vital signs, thoroughly examines a patient from head to toe, and submits orders, like urine samples or blood tests, the Medicare Annual Wellness Visit does not provide such a thorough exam. Rather, it focuses on prevention planning and reviewing the medical history.

Instead of being guided by hands-on measurements, the Medicare annual wellness visit is driven by the health risk assessment. The HRA is a questionnaire and screening tool that provides an assessment about health status and provides feedback about actions that can be taken to reduce risks and promote health. Patient communication when performing the health risk assessment is key.

The provider works with patients to develop a personalized prevention plan, which requires the following:

    • medical and family history
    • treatment options for risk factors and
    • development of a screening schedule.

In addition, providers may assess for cognitive impairment or look for signs of Alzheimer’s disease or dementia. Medicare patients pay nothing for the annual wellness visit however, they must be eligible for the service. Medicare.gov notes that beneficiaries with Medicare Part B for longer than a year are eligible once every 12 months.

A Medicare Annual Wellness Visit Vs An Annual Physical Exam

Last year, an article ran in the Washington Post about a Medicare beneficiary who scheduled her annual checkup with her primary care doctor. To her, the appointment seemed like a routine physical, and she assumed that Medicare would pay for the visit until she received a large bill.

Why did this happen? There is a difference between an annual wellness visit and an annual physical exam. One is focused more on preventing disease and disability, while the other is more focused on checking your current overall health. Wed like to clear up any confusion you might have.
