When Does Medicare Cover Emergency Medical Transport Services

Medicare covers medically necessary medical transportation to the closest hospital in the event of an emergency. Medicare Part B generally pays all but 20% of the Medicare-approved amount for most doctor services plus any Part B deductible. Ambulance companies must accept the Medicare-approved amount as payment in full. This also applies to emergency air medical transport services. If Medicare determines your condition did not warrant emergency medical transportation, it may not cover any of the costs.

In some very limited cases, Medicare will also cover non-emergency medical transport services by ambulance, but you must have a written order from your health-care provider stating that your medical condition requires transportation by ambulance.

Keep in mind that Medicare is testing a new program in a few states for beneficiaries who need scheduled, non-emergency medical transportation three or more times in a short period. In these states, the ambulance company is required to get prior authorization before a fourth ride is arranged if Medicare denies authorization, and you still use the ambulance, the company may bill you in full for all charges. States and districts currently affected by the program include:

Does Medicare Cover Power Lift Chairs

According to Medicare Part B, coverage may be applicable to a portion of the lift chair as Durable Medical Equipment under particular circumstances. A lift chair unit consists of two main components, the chair itself and the motorized lift mechanism. Vertical Vs Inclined Platform Lifts

There are two types of platform lifts: vertical platform and inclined platform:

Vertical platform lifts, which can extend up to 14 feet, are most commonly installed in homes, as well as outside to access porches. They are similar to elevators, moving straight up and down, but are more cost efficient. Unlike elevators, vertical platform lifts are not fully enclosed, though a cage often encloses the platform of the lift. For installation, a 6-foot square area of space is required. Once youve met the deductible, youll pay 20 percent of the Medicare-approved amount for the lift mechanism. You will also pay 100 percent of the remaining cost of the chair.

Medicare Advantage plans have been allowed, up to this point, to offer some extra benefits which are for vision and hearing, but in the next year they will be allowed to add new benefits which can be tailored to peoples illnesses, so there may be greater coverage for lift chairs.

When Does Medicare Cover Adjustable Beds

Medicare covers an adjustable bed when you have a specific medical need for it. You must have a face-to-face visit with your doctor or provider within 6 months of your doctor placing the order for the bed.

You must also be enrolled in Medicare Part B, which is part of original Medicare, to have an adjustable bed or any other durable medical equipment covered.

To be considered for coverage, you must have the following:

  • a condition that requires special equipment or positioning not possible with a standard bed
  • a prescription from your doctor for an adjustable bed that describes your medical condition and why it requires an adjustable bed
  • medical records that support the need for an adjustable bed
  • physician reports that detail the medical condition for which an adjustable bed is needed

Some conditions that might cause you to need an adjustable bed include:

  • those that require frequent positioning of your body to address pain, contractures, or poor alignment

Level Of Care Required

To be eligible for a waiver, the recipient must meet the Level of Care Required threshold. Level of Care Required means that an individual must be eligible to receive a specific level of care through regular Medicaid to utilize a waiver program. For instance, someone who receives Medicaid but is capable of living with their family or on their own, often does not meet the required level of care. However, a disabled individual who is unable to remain at home due to a disability or worsening health would be deemed Nursing Home Eligible or Eligible for Institutional Level of Care. If equipment such as a stair lift or skilled home care allows the individual to remain at home, they are likely eligible for a waiver.

Alternatives To Stair Lifts

Stair lifts, without financial assistance, can be beyond the reach of many families. Unfortunately, there are very few less expensive alternatives. One option is to install handrails on both sides of the staircase. This narrows the staircase considerably, allowing stair climbers to stabilize themselves from both sides simultaneously. The handrails should extend one foot beyond the top and bottom of the stairway. Though obviously not perfect, this solution should cost less than $200 installed.

A second option is sliding stair rails. These are grab bars, which slide and lock into position in front of the stair climber and move along as they climb or descend. One manufacturer of these products is called StairAid. StairAids, though still fairly expensive, cost approximately 50 percent less than an electric stair lift.

A third option is Stair Blocks. These are blocks approximately one-foot-wide, the width of a stair and one-half the height of a stair. A block is placed on each step and when a user climbs, they alternate between the regular stair and the block. Stair blocks make climbing easier, but the individual still must climb and may still experience a fall. See a video.

Lastly, there are vertical or inclined platform lifts, perhaps better thought of as elevator lifts. Typically, these cost more than stair lifts but can accommodate persons confined to wheelchairs. Learn more.

The VA provides grants to veterans and service members with certain service-connected permanent disabilities. These grants are available to help buy or build an adapted home, or modify an existing home to accommodate a disability.

The Specially Adapted Housing grant helps veterans with certain disabilities live independently in a barrier-free environment. SAH grants can be used to remodel an existing home if it can be made suitable for specially adapted housing.

According to the VHA Handbook, stairlifts may be furnished as part of medical and/or prosthetic services to eligible veterans provided that they are medically necessary for the veterans care and treatment.

How To Prove Medical Necessity

One way to show medical necessity is to have your doctor prescribe an in-home assessment as part of your plan of care for an illness or injury. The evaluator should also look at your abilities to recommend certain modifications as part of the medical intervention.

The American Occupational Therapy Association describes three scenarios and how Medicare is likely to respond to each.6

In the first scenario, a medical provider asks an occupational therapy office to assess a client at home. The client has undergone a recent serious illness that decreased their functional performance. An occupational therapist goes to the home for a functional assessment and home environment review. The therapist makes recommendations for certain home modifications as part of the medical intervention. Medicare covers the evaluation itself and could subsidize the recommended modifications because theyre part of the occupational therapy plan of care.

The letter should include:

  • Diagnosis information
  • The specific features offered by the particular tub prescribed and how the tub addresses your medical need
  • The duration of treatment
  • The specification that alternative treatments cost more, do not exist or do not work as effectively

Its possible to get coverage for a condition you expect to have for the rest of your life, but your doctor needs to regularly submit a letter of medical necessity.
