Why Would I Need To File A Claim

Community Health Network accused of filing false Medicare claims

As we mentioned, you may not need to file a claim if youre on Medicare. Most of your claims will be filed for you if youre on Original Medicare, and most Medicare Advantage plans dont use claims. But there are some situations youll need to know about.

If its approaching 12 months since you received a service and your doctor, provider or supplier still has not filed a claim. Call your doctor, provider or supplier to see if they will file the claim. If they are unable or refuse, you may need to file a claim yourself. Medicare suggests you call 1-800-MEDICARE to find out the exact date a claim must be received by.

If you receive care from a doctor or provider that doesnt accept Medicare assignment. Medicare still may pay its portion, but youre on your own to do the leg work.

You received medical care outside the U.S. Medicare will cover claims made in specific situations:

  • You have a medical emergency while in the U.S. but a foreign hospital is closer than the nearest U.S. hospital.
  • You have a medical emergency while traveling through Canada via the most direct route between Alaska and another state. If this happens and a Canadian hospital is closer than a U.S. hospital, Medicare may cover care at the foreign facility. These trips must be made without reasonable delay, which Medicare judges case by case.
  • You live in the U.S. and the nearest foreign hospital is closer than the nearest U.S. hospital.

Different Categories Of Social Security Benefits

Over 1.6 million Pennsylvania retirees receive a payment from Social Security each month on their own earning record. But Social Security is much more than just a retirement program for workers. Nationally, only about 64% of beneficiaries receive their payments as retired workers.

Another 4.2% receive benefits as non-working spouses based on the retired workers earnings. The remaining 32% receive benefits as disabled, survivors, or dependents.

Social Security assigns letter codes to identify under which category a recipient is qualifying for his or her benefit. The Social Security website has a list of the most frequent codes you can find it here.

My friend qualified for Social Security on his own record as a wage earner. Thus the letter on his Medicare card is A the most prevalent code. His wife did not work outside the home. She qualified for Social Security benefits as the spouse of a retired wage earner, based on her husbands record. Thus, her card uses the letter B along with the Social Security number of the wage earner her husband.

Things To Know About Your New Medicare Card

Medicare Hospital Stay 3 Days: What Is A Medicare Claim Number

Got Medicare? Read on!

Until now, losing your Medicare card was as significant a problem as losing your Social Security card. Thats because your Medicare card included your Social Security number.

You know how important your Social Security number isand how you never want it to fall into the wrong hands. Its tied to your identity in a lot of ways, and in the wrong hands, it could lead to identity theft.

A new lawthe Medicare Access and CHIP Reauthorization Act of 2015required Medicare to remove Social Security numbers from Medicare cards. The existing Health Insurance Claim Number on cards, which is based on Social Security numbers, will be replaced with whats called the Medicare Beneficiary Identifier, or MBI.

This month, the U.S. government began the process of mailing Medicare users the new card. The idea of replacing the Social Security number with the MBI is to help keep your personal information more secure and help protect your identity.

The changeover wont happen overnight. In fact, the government will mail out the cards over the course of a yearfrom April 2018 through April 2019.

What will the new Medicare card look like? The new Medicare card will still be white, with red, blue, and black print, but it will sport a different design, as shown in the samples below from the Centers for Medicare & Medicaid Services website. The new card will be made of paper, not plastic. This is intended to make the card easier for providers to use and copy.

Claim Status Request And Response

Providers have a number of options to obtain claim status information from Medicare Administrative Contractors :

Providers can enter data via the Interactive Voice Response telephone systems operated by the MACs.

Providers can submit claim status inquiries via the Medicare Administrative Contractors provider Internet-based portals.

Some providers can enter claim status queries via direct data entry screens.

Providers can send a Health Care Claim Status Request electronically and receive a Health Care Claim Status Response back from Medicare.

The electronic 276/277 process is recommended since many providers are able to automatically generate and submit 276 queries as needed, eliminating the need for manual entry of individual queries or calls to a contractor to obtain this information. Submission of 276 queries and issuance of 276 responses should be less expensive for both providers and for Medicare. In addition, the 277 response is designed to enable automatic posting of the status information to patient accounts, again eliminating the need for manual data entry by provider staff members. If unsure whether your software is able to automatically generate 276 queries or to automatically post 277 responses, you should contact your software vendor or billing service.
