How To Cancel Medicare Part B
The Part B cancellation process begins with downloading and printing Form CMS 1763, but dont fill it out yet. Youll need to complete the form during an interview with a representative of the Social Security Administration by phone or in person.

During your interview, fill out Form CMS 1763 as directed by the representative. If youve already received your Medicare card, youll need to return it during your in-person interview or mail it back after your phone interview.

If You Already Have Part A But Not Part B
If you enrolled previously in Part A but not Part B then you have the option to apply for Medicare Part B online using a special application link at the Social Security website. You can also upload the two required forms with your online application that shows proof of creditable coverage.
To start the application, scroll down to the bottom of the page and check the box that you understand and agree.
Then click Start Application. This is an electronic form so click the yellow arrow that says Start. Then fill in the required information on the form and scroll down the page a bit where youll see a remarks box.
Type in the first of the month that you want your Part B to start. Fill in the employers information. And then at the bottom you can attach the two forms that are required to be sent in with your application. Youll click to Sign and youll also need an email address to confirm your digital signature.
Youll need to receive and complete the email confirmation before the application is submitted.You still have the option to send in your forms by Fax 1-833-914-2016 or mail to one of the Social Security Field offices.
Social Security will process your Medicare Part B application and issue you a new Medicare card that includes your added Part B effective date on it.

Aging In To Medicare Coverage
