Getting Your Medicare Card: Who, What, When, Where & How

If your baby was born in an Ontario hospital with birthing facilities or with a registered midwife, follow these steps to apply:

Complete the Ontario Health Coverage Infant Registration form. It is not available online. Hospital staff or the registered midwife will give you the form. On the form you must:

give your babys name, birthdate and mailing address

confirm your babys primary place of residence is Ontario

confirm your baby will be in Ontario for at least 153 days in any 12month period

The hospital or midwife will submit the top part of the form to us on your behalf.

Keep the bottom section of the form until you get your babys Ontario health card. It shows the health number assigned to your baby.

Your babys new health card will be mailed to you within 8 weeks of the date the Ontario Health Coverage Infant Registration form was submitted.

If hospital staff or the registered midwife gave you the Ontario Health Coverage Infant Registration form and you have questions or concerns about your babys health card, you must contact ServiceOntario.

If hospital staff or the registered midwife do not give you the Ontario Health Coverage Infant Registration form, you may apply for health coverage for your child at either:

How To Apply For Medicare

How Do I Replace A Lost Medicare Card

You can order a replacement Medicare card online, using the Social Security website. Youll be directed to log into your My Social Security account, where youll be able to request a new card. The card will be mailed to the address on your account within 30 days so make sure your address correct. You can also call Social Security toll-free at 1-800-772-1213 between 7 a.m. and 7 p.m. If you are deaf or hard of hearing, call Social Securitys toll-free TTY number at 1-800-325-0778.

If you need proof that you have Medicare sooner than 30 days, you can request to have a letter mailed to you within 10 days. Once your application is processed, you will receive a notification letter in the mail to inform you of whether your application was approved.

During Your Initial Enrollment Period

This lasts for seven months, of which the fourth one is the month in which you turn 65. For example, if your 65th birthday is in June, your IEP begins March 1 and ends Sept. 30.

To avoid late penalties and delayed coverage, you need to sign up for Medicare during your IEP in these circumstances:

You have no other health insurance

You have health insurance that you bought yourself
You have retiree benefits from a former employer
You have COBRA coverage that extends the insurance you or your spouse received from an employer while working

You have veterans benefits from the Department of Veterans Affairs health system
Youre in a nonmarital domestic relationship with someone of the same or opposite sex and you are covered by his or her employer insurance

If you enroll during the first three months of your IEP, your Medicare coverage begins on the first day of the month you turn 65 . If you sign up during the fourth month, coverage begins on the first day of the following month. But if you leave it until the fifth, sixth or seventh month, coverage will be delayed by two or three months. For example, if your birthday is in June and you sign up in September , coverage will not begin until Dec. 1.
