    What Is A Chronic Condition Special Needs Plan

    A Chronic Condition Special Needs Plan, or C-SNP, is a type of Medicare Advantage Plan for people with chronic conditions. These conditions include:

    • Mental health issues

    C-SNPs are a type of Medicare Advantage Plan geared specifically for patients with these types of long-term illnesses. Because they are looking at a lifetime of medical care, typical out-of-pocket expenses are often waived. Benefits include drug prescription plans like Medicare Part D, as well as health and wellness programs, substance abuse counseling and diabetes management services. Dental, vision and hearing coverage is sometimes included.

    Many C-SNP plans come with low to no monthly premiums. Plans include both HMOs and PPOs, and have fewer coverage limitations than the typical Medicare Advantage Plan. To be eligible for a C-SNP plan, you must:

    What Are Dual Special Needs Plans (D-SNP)?

    Allwell offers a few kinds of Medicare Advantage plans, and they vary in structure, costs and benefits.

    In general, Allwell offers Medicare Advantage Prescription Drug plans, or MAPDs, as well as Medicare Advantage Plans without drug coverage.

    Plan availability may vary by county. Other plan offerings may include the following types:

    A health maintenance organization, or HMO, generally requires that you use a specific network of doctors and hospitals. You may need a referral from your primary doctor in order to see a specialist, and out-of-network benefits are usually very limited.

    Preferred provider organization, or PPO, plans provide the most freedom, allowing you to see any provider that accepts the insurance. You may not need to choose a primary doctor, and you dont need referrals to see specialists. You can seek out-of-network care, although it may cost more than seeing an in-network doctor.

    Special needs plans restrict membership to people with certain diseases or characteristics. Hence, the benefits, network and drug formularies are tailored to the needs of those members. Allwell offers two types of SNPs:

    • Chronic Condition SNP: For people with one or more specific conditions, such as chronic heart failure, congestive heart failure or diabetes.

    • Dual-Eligible SNP: For people who are entitled to Medicare and who also qualify for assistance from a state Medicaid program.

    Individuals Covered By Both Medicare And Medicaid

    AHCCCS continues developing integration initiatives to increase alignment and improve service delivery for individuals covered by both Medicare and Medicaid. These individuals, commonly referred to as âdual eligible members,â navigate multiple health systems to receive care. This health system fragmentation often results in poor communication, uncoordinated health care decisions and a lack of a patient-centered perspective. AHCCCS has moved toward increasing the coordination of health service delivery between these two health programs by contracting with Medicare Advantage Dual Special Needs Plans that are each affiliated with its partner AHCCCS Complete Care Medicaid health plan. Requiring each ACC Medicaid health plan to offer a partner Medicare D-SNP promotes the enrollment or alignment of dual eligible members in the same health plan for both Medicare and Medicaid services to the greatest possible extent. Enrolling in specialized Medicare plans allows dual eligible members to receive all of their health care services, including prescription drug benefits, from a single, integrated health plan.

    Depending on where they live, Allwell customers can get a plan costing between $0 and $286 per month. But while $0 premium policies are available, customers as a whole are not happy with the cost of their Allwell plans.

    Allwell customers are generally happy with their service based on an average Centers for Medicare & Medicaid Services rating of 3.5 of 5 stars. But Allwell did not score well with other entities like the National Committee for Quality Assurance and the Better Business Bureau.

    Allwell Medicare Advantage members are generally happy with the company’s customer service but not with plan costs. In fact, cost was the main reason customers dropped their Allwell coverage.

    Allwell rates range from $0 to $286 per month, with at least one $0 plan sold in each state where the company does business. But in some areas, the only premium-free options are special needs plans. Most Medicare enrollees in those areas don’t have a no-cost Allwell plan to choose from.
