Using My Medicaid Card
It is important to show each health care provider your Medicaid Member Card BEFORE you receive any services. To avoid problems, carry the cards listed below with you each time you seek health care services.
Medicaid Member Card: Show your Medicaid Member Card BEFORE each appointment and BEFORE you order medical supplies, prescriptions, hearing aids, etc. When you show your card before you get medical care, you can find out if the health care provider accepts Medicaid and whether the service is covered.
Health Plan Card: If you are also enrolled in a health plan, your health plan may also send you a separate member card with instructions. Show both your Medicaid Member Card and the health plan card to each medical provider. Then you can find out if the services you will receive are covered by Medicaid.
Picture I.D. Card: When you see a health care provider for the first time, you may be asked to show a picture identification card as well. This is to prevent someone else from using your Medicaid or health plan cards to get health care services. Neither Medicaid nor your health plan will pay a provider for services given to a patient who was not covered.
Printing Out A Replacement Card
If your card has been lost or damaged, you can print out a copy via your MyMedicare.gov account. This will allow you to print an official copy of your Medicare card that you can carry with you and use when you go to the doctor.
Although this isnt technically a Medicare card, the information will be the same as what is found on your card. This means that anywhere that you can use your real Medicare card, you can also use this printed out version.
Getting An Extra Card
Your family can get a second card with the same details. We provide an extra card when more than one family member needs a card. We can only issue one extra card.
If someone in your family is 15 or older, they may want to get their own card. Read more about getting your own Medicare card. We cant issue an extra card when youre the only person on your card.

If You Do Not Qualify For Automatic Enrollment Youll Get Your Medicare Id Card Once You Complete Enrolling In Original Medicare Youll Need To Enroll Yourself If Any Of The Below Are True For You
- If you are turning 65 and are not receiving Social Security or Railroad Retirement Board benefits. Coverage usually begins the first day of your birthday month. If your birthday is on the first day of the month, coverage usually starts the month prior.
- If you have End-Stage Renal Disease Permanent kidney failure requiring dialysis or a kidney transplant., you will receive your Medicare ID card once you complete your Original Medicare enrollment. Coverage starts the first month your disability begins.
- If you live in Puerto Rico and receive social security benefits from Railroad Retirement Benefits, you are automatically eligible for Part A. If you also want Part B, you need to apply for it. You will receive your new Medicare ID card once you complete your Part B enrollment. Fill out this Application for Enrollment in Part B.
Will I Automatically Get My Medicare Card In The Mail
Once you’re signed up for Medicare, we’ll mail you your Medicare card in your welcome packet. You can also log into your secure Medicare account to print your official Medicare card. How To Replace Private Medicare Plan Cards
Those who are enrolled in Medicare Advantage plans, Medicare Part D Prescription Drug plans, Medicare Supplement plans or other types of Medicare plans may receive separate insurance cards for those.

Make An Online Request
Many states also make it possible to order a replacement card online. Go to the Medicaid page of your states health and human services website. If you dont immediately see this section, try typing in Medicaid replacement card in the search box and click search. The first or second link in the search results likely will be the page youre looking for. Next, fill in the information required. For example, on New York Citys website, you select the reason youre requesting a replacement card from the drop-down menu labeled Details. Your choices include lost, stolen and damaged. Next, you enter your Social Security number. If you dont know it, click I dont know it icon and move to the next section. If the card is for your unborn child, click Request is for an unborn child icon. Next, click the right arrow key at the bottom of the page to to submit your request. How To Print Or Request A New Medicare Card Online

Protect Yourself When You Carry Your Physical Card
When carrying your Medicare card or a printed copy of your card make sure to keep it in a safe and secure place. Storing your card in a coat pocket or in the back of your mobile phone may not be the best idea.
If you have Original Medicare then you need to carry your card whenever you are away from home. When you go to the hospital, doctors clinic, or a health care provider you will need to show your Medicare card. If you have a Medicare plan that covers prescriptions or supplementary services carry that plan information or card with you as well.
If you have a Medicare Advantage plan or any other kind of Medicare plan you will need to use your specific plans card to get services, you will not need to show your Medicare card.
Make sure to keep any and all health care plans, coverage proof, and plan cards in a safe place. Secure the documents so water will not damage them and keep them in a safe place you will not forget.
You should also photocopy all of your important documents and cards so if they are lost you have a copy and can prove you are the owner of the documents. You should also write down all your important identification numbers for your reference.

If you dont already have an account, you can create one online. Go to . Once you are logged in to your account, select “Replace your Medicare Card” link under the Medicare Enrollment Detail section. Then select Mail my replacement Medicare Card. Your Medicare card will arrive in the mail in about 30 days at the address on file with us. What If I Pay Part Of My Income For The Cost Of My Long
If you are enrolled in a PDP where you have to pay part or all of your copays and premium, it is considered a medical expense and may be used to reduce your participation.
Report the amount you pay for copays and premiums to your local Home and Community Services financial worker. If you dont remember who your financial worker is, call your local HCS Office. The financial worker will need to see receipts for the amount you report.
If you get food assistance, copays and other medical expenses that exceed $35.00 per month may be used to give you more food benefits. Report the amount you pay to your financial worker and provide receipts for the amount you report.
