Following Anterior Cruciate Ligament Reconstruction
The day after the procedure, the patients were randomized to receive a single injection of 3 mL HA or 3 ml saline solution after surgical drains were removed. All patients were evaluated at baseline and at 15, 30, 60, and 180 days and 12 months after surgery by use of the following tools: Short Form-36 Health Survey , International Knee Documentation Committee subjective score, VAS for pain, VAS for general health status, and Tegner score. At each follow-up evaluation, the trans-patellar circumference and active and passive ROM of both knees were recorded. No severe adverse events were documented after early viscosupplementation. A significant improvement was documented in both treatment groups. Significant differences were documented in the trans-patellar circumference at 60 days and in active ROM at 30 days post-operatively patients who received HA had better values compared with the placebo group . No statistically relevant intergroup differences were found in the clinical scores. The authors concluded that he findings of this study documented no AEs and had some positive findings in terms of active ROM recovery and trans-patellar circumference reduction. However, the early post-operative application of viscosupplementation did not lead to significant improvement in clinical scores after ACL reconstruction.

Combined Use Of Viscosupplementation And Other Intra
In a randomized clinical trial, de Campos et al addressed 3 questions: Does the addition of triamcinolone to viscosupplementation
These researchers prospectively enrolled 104 patients with knee osteoarthritis and randomized them to receive either a single intra-articular injection of hylan GF-20 , or a single intra-articular injection of hylan GF-20 and 1 ml of triamcinolone hexacetonide . VAS, WOMAC, and Lequesne questionnaires were completed at baseline and at weeks 1, 4, 12, and 24. At week 1 the WOMAC and VAS scores were lower in Group VS + T, compared with Group VS. There was no difference regarding the adverse effects. At weeks 4, 12, and 24 there were no differences in the groups. The authors concluded that the addition of triamcinolone hexacetonide improved 1st-week symptom and functional scores of viscosupplementation, but not beyond. It did not appear to increase the likelihood of adverse effects. They also stated that one can speculate that triamcinolone could positively affect the action in disease progression however, more studies are needed on this matter.

To make sure hylan G-F 20 is safe for you, tell your doctor if you have:

- an allergy to birds, feathers, or egg products.
It is not known whether hylan G-F 20 will harm an unborn baby. Tell your doctor if you are pregnant or plan to become pregnant.
It is not known whether hylan G-F 20 passes into breast milk or if it could harm a nursing baby. Tell your doctor if you are breast-feeding a baby.
Hylan G-F 20 is generally not used in anyone younger than 21 years old.

Specialty Pharmacy Update: Changes To Select Medication List Effective June 1 2021
National Drug Codes and NDC data are required by Blue Cross and Blue Shield of Illinois for drugs billed under the medical benefit on professional/ancillary electronic and paper claims. When you bill with NDCs, BCBSIL can identify and reimburse you for the individual medication that was prescribed .
With this capability in mind, BCBSILs Select Medication List includes specific drugs within particular classes or categories, based on safety and cost effectiveness. Choosing a drug from the Select Medication List and billing for it with the appropriate NDC may result in a higher reimbursement rate than the rate applied for other, non-listed drugs in the same class.
The Select Medication List is posted in the Specialty Pharmacy section, under the Related Resources. Currently, it includes two injections from the viscosupplementation class that are used primarily for osteoarthritis knee pain relief. Upcoming changes are noted below.
The products on the Select Medication List will be updated effective June 1, 2021.
Euflexxa® will be removed as a viscosupplement from the Select Medication List. It will be replaced by Orthovisc®. Synvisc-One® will remain on the Select Medication List. Please note reimbursement may change to reflect these product changes.

As defined in Aetna commercial policies, health care services are not medically necessary when they are more costly than alternative services that are at least as likely to produce equivalent therapeutic or diagnostic results. There are several viscosupplement products on the market. There is a lack of reliable evidence that any one viscosupplement product is superior to the other for medically necessary indications. Orthovisc , Synvisc , and Synvisc One brands of viscosupplement are less costly to Aetna. Consequently, because other viscosupplement products , Euflexxa , Gel-One , Gelsyn-3 , GenVisc 850 , Hyalgan , Hymovis , Monovisc , Supartz FX , Synojoynt , Teva sodium hyaluronate 1% , Triluron , TriVisc , and Visco-3 ) are more costly than the alternative lower cost viscosupplement products that are at least as likely to produce equivalent therapeutic results as to the treatment of the member’s disease, the more costly viscosupplement products will be considered medically necessary only for members who have a contraindication, intolerance or ineffective response to the available equivalent alternative viscosupplement products: Orthovisc, Synvisc, and Synvisc One.
