Having health insurance does not always mean you can afford to use it. There may be expensive deductibles to pay out of pocket before your insurance coverage kicks in. When insurance does pay for services, you could still be left to pay copays or coinsurance .

Not everyone is fortunate enough to afford health insurance. As many as 17% of adults between 60 and 64 are on Medicaid, and 8% are uninsured. States where Medicaid expansion did not occur tend to have higher rates of uninsured people.

Unfortunately, many people delay care due to concerns over cost. In 2019, 9.1% of people delayed getting the care they needed, and 8.3% chose not to get care at all. When it came to people who did not have insurance, those rates increased to 32.2% and 30.4%, respectively.

A study in the journal Cancerlooked at a national database of cancer diagnoses from 2014 to 2016. The researchers focused on uninsured adults between 61 and 64 years old and compared them to insured adults on Medicare from 65 to 69 years old.

Interestingly, there was a significant rise in cancer diagnoses at 65 years old, especially for colon cancer and lung cancer. The implication is that people delayed care until they qualified for Medicare.

Medicare at 60 could be an option for Americans who do not qualify for Medicaid or who cannot otherwise access affordable health care.

Can You Get Medicare Supplement Insurance If You Are Under 65

Federal law doesnt require insurance companies to sell Medicare Supplement Insurance for disabled people under age 65, but some state laws do. If youre enrolled in Medicare under 65 due to a disability and/or end-stage renal disease , your eligibility for Medicare Supplement Insurance will depend on the state you live in.

The following states require that insurance companies offer at least one type of Medigap plan to people under 65 who receive Medicare benefits:

  • California*, Colorado, Connecticut, Delaware**, Florida, Georgia, Hawaii, Illinois, Kansas, Louisiana, Maine, Maryland, Massachusetts*, Michigan, Minnesota, Mississippi, Missouri, New Hampshire, New Jersey, New Mexico, New York, North Carolina, Oklahoma, Oregon, Pennsylvania, South Dakota, Tennessee, Texas, Vermont* and Wisconsin.

*These states require that insurance carriers offer at least one Medigap policy to those under 65 who are disabled. This requirement does not include those under 65 with ESRD.

**Delaware requires that insurance carriers offer at least one Medigap policy to those under 65 who have ESRD. This requirement does not include those under 65 who are disabled.

Even if your state isn’t listed above, you may be able to get coverage.

Some insurance companies voluntarily sell Medicare Supplement Insurance plans for disabled people who are under 65. If they do, they generally use medical underwriting in the application process, which could affect your Medigap policy cost.

  • You cant get premium tax credits and other savings based on your income. This is true only if youre actually enrolled in retiree coverage. If youre eligible for but not enrolled in retiree coverage, you may qualify for premium tax credits and lower out-of-pocket costs based on your household size and income.

  • If you voluntarily drop your retiree coverage, you wont qualify for a Special Enrollment Period to enroll in a new Marketplace plan. You wont be able to enroll in health coverage through the Marketplace until the next Open Enrollment period.

From 10,000 feet up, the idea of lowering the age is good news for some. Ive had clients who were able to save large sums and could financially afford to retire but felt like they had to keep working until they could get guaranteed health coverage at age 65 through Medicare.

And then there are those who are forced into early retirement. If you lose your job and are over 55, finding a new job isnt always easy. So what do you do for healthcare at that point?

The plans you can buy on the healthcare exchange plans are very expensive for older Americans. This is where access to Medicare at 62 or even younger ages could help.

According to one study, a typical 60-year-old could buy into Medicare for about $8,000 less per year as compared to a gold-level plan on the insurance exchanges, and $3,700 less than buying a bronze-level plan.

A 2019 Kaiser Family Foundation poll also found most Americans support expanding Medicare coverage. 77% of respondents support the idea of introducing a Medicare buy-in for people as young as 50. This support seems bipartisan 85% of Democrats and 69% of Republicans polled approved of the idea.

At face value, it seems that lowering the eligibility age could be the perfect solution that provides the most options for individuals who are nearing retirement age.

But despite the broad idea of this proposal having public support, there are a few roadblocks that any of these proposals to lower the age will likely meet.

The Challenges Of Changing Medicares Age Of Eligibility

First, there are the funding issues. If lowering the age to access increased the cost to administer the Medicare program, the chances of proposed changes passing will drop.

The Part A account that funds the hospitalization and related services faces insolvency by 2026. Insolvency means that Medicare wouldnt be able to fully reimburse hospitals, nursing homes, and home health agencies for promised benefits.

If this happens, Medicare patients would be hit hard. Theres no way around this. You cannot cut provider payments for medical services without impacting the beneficiaries of those services.

In response to the funding concerns, the Democrats who are sponsoring this bill say that Medicares funding wouldnt be touched under the proposed legislation since those buying the coverage would be required to pay the entire cost.

That means whatever the cost is to the government, would also be the cost to the individual.

Second, youre going to get lots of pushback from healthcare and health insurance companies. Theyll spend millions to lobby against this, which could impact the outcome of the law changes.

From the hospitals perspective, theyll be losing millions of Americans from their most profitable group of patients: Those who are 50 and up and covered by private healthcare. There is a big difference between the reimbursement rate between a Medicare patient and a patient with private health insurance.

If You Get Medicare For Disability And Then Return To Work

Will My Spouse of 62 Receive Medicare if I’m 65 and I Receive Medicare Benefits?

If you get Medicare due to disability and then decide to go back to work, you can keep your Medicare coverage for as long as youre medically disabled.3 And, if you do go back to work, you wont have to pay the Part A premium for the first 8.5 years.

Part A is premium-free for those with a disability and under 65 only if you get Social Security or Railroad Retirement Board benefits for 24 months or have ESRD and meet certain requirements.4

If youre 65 or older, Part A is premium-free if you or your spouse worked and paid Medicare taxes for at least 10 years, you already get retirement benefits from Social Security or the Railroad Retirement Board, or youre eligible for these benefits but havent filed for them yet.5

Enrolling In Medicare With A Disability

Just like when you become eligible for Medicare at age 65, when you are eligible with disability, you have an Initial Enrollment Period of 7 months.

Your Initial Enrollment Period will begin after you have received either disability benefits from Social Security for 24 months or certain disability benefits from the Rail Road Retirement Board for 24 months.1 In other words, your IEP starts on the 25th month of disability benefits.

You will be automatically enrolled in Medicare Part A and Part B, but if you decide you want to get a Medicare Advantage or Part D prescription drug plan, you will need to enroll yourself directly with the private plan provider. You will need to enroll during your IEP to avoid late enrollment penalties.

Medicare For Divorced Spouse

The Social Security Administration requires you to meet specific criteria to qualify for Medicare benefits from a divorce. Qualifying for Medicare is different than Social Security benefits. You can be eligible for your spouse/ex-spouse Social Security benefits at age 62, and you wont qualify for Medicare until age 65. Of course, you may be eligible for Medicare sooner if you have End-Stage Renal Disease or disability for at least two years.

If youre 62 and your spouse or ex-spouse is 65, you CANNOT use their Medicare benefits for eligibility. You must wait until the age of 65 to qualify unless youre eligible through disability.

If the following situations apply, you may qualify for Medicare after divorce:

  • Your ex-spouse is at least 62 years old and eligible for Social Security.
  • You must be currently unmarried.
  • Youre at least 65 years old.
  • You were married for 10+ years.

Part A benefits are free when you, a current or former spouse, have at least 40 calendar quarters of work or ten years of work history paying into Social Security.
