What Is An Enlarged Prostate
The prostate tends to get larger as men age. This enlargement often squeezes the urethra, which is the internal pathway tube that carries urine from the bladder to the outside the urethra passes right through the prostate.
The squeezing can create a blockage of the urine flow. The symptoms of BPH can include difficulty starting urination, a reduced stream, increased need to urinate, and the inability to completely empty the bladder.
If left untreated, BPH can develop into urinary retention or incontinence, bladder stones, and damage to the bladder and kidney.

A Treatment Option For Men With Enlarged Prostates
Its a procedure that can improve urinary symptoms in men with enlarged prostates — without surgery or the removal of tissue. And it doesnt cause sexual side effects.
The UroLift is a new option for some men. The urologist pulls the sides of the prostate away from the urethra, using steel sutures to hold the prostate tissue back – like the tiebacks that keep window draperies open . This is meant to widen the urethra to allow for a better flow of urine.
What I usually tell my patients is that we are basically Moses opening the sea. We are creating a nice channel for urine to flow, says Hanan Goldberg, MD, an assistant professor of urology at Upstate.
The sutures remain for life. Goldberg says they meld into the prostate tissue within two or three weeks. Men who feel burning, frequency or urgency after the procedure usually see those side effects go away within two to four weeks. He says the procedure can improve a mans quality of life. Success rates after five years are about 87 percent.
Goldberg says UroLift does not affect a mans ability to have an erection or to ejaculate normally. Other treatments for benign prostatic hyperplasia might involve those sexual side effects.

Cost Of Urolift Procedure: Considerations
There are two parts to consider when discussing the cost of the Urolift procedure.
The overall cost of the Urolift procedure is made up of a few components. The first part is a professional fee charged by the urologist performing the procedure. If the Urolift procedure is performed in the office settings, this charge will typically include facility costs, nursing costs, and supply costs used to perform the procedure.
If the procedure is performed in the hospital or surgical center, a is typically charged by the hospital or surgical center for facility use. In addition, another fee will be billed by the anesthesiologist for anesthesia.
It is obviously less expensive to have the procedure performed in the office under local anesthesia. You save on hospital facility fees, which can be anywhere from $3,000-$15,000. You also save on the anesthesiologist fee.
The other cost consideration is the actual cost of Urolift implants. A typical patient will need anywhere from 6-8 Urolift implants. PAE has been shown to have fewer side effects than other widely used treatments for BPH such as transurethral resection of the prostate surgery, which is associated with incontinence, impotence, retrograde ejaculation and erectile dysfunction issues in men. When the procedure is performed by an experienced physician, the complication rate is extremely low and the procedure is incredibly safe and effective. According to the World Journal of Urology, the following side effects can occur following PAE: transient blood in urine , rectal bleeding, and acute urinary retention. What Happens During The Pae Procedure
It works by placing a tiny catheter into the arteries that feed the prostate gland. A solution containing thousands of microscopic plastic beads is injected these block the blood supply by shutting down blood flow to the gland. All of this is accomplished through a tiny quarter-inch hole in the skin. General anesthesia is not needed because the procedure is not all that painful. Most patients go home the same day with just a small bandage, requiring only a few days of recovery at home before returning to normal life. Over time, the prostate gland is starved of its blood supply, and it shrivels up, like a grape shrinking into a raisin, says Dr. Ayyagari. This allows the urethra to open up and urine to flow more normally. Amazingly, shrinking the prostate does not have any negative impact on sexual function, and indeed a small number of patients actually experience an improvement in their sexual function after the procedure.
Who Is A Good Candidate For The Urolift System Treatment

Q: Can My Prostate Be Too Large For Urolift
A: In a Q& A with Dr. Daniel Rukstalis told Harvard Health: Its FDA-approved for men 45 and older with prostates up to 80 grams in size . But my view is that UroLift works best in prostates ranging from 25 to 60 grams. About a third of men with BPH also have whats called a median lobe, or a bit of prostate tissue that protrudes up into the bladder. We just completed a clinical trial showing that UroLift works well for these men too. On the basis of that study, the FDA approved UroLift for men with median lobes in early 2018. Well typically evaluate potential candidates with a pelvic ultrasound, which provides a lot of information about the health of the bladder and the size and shape of the prostate.

Who Is A Candidate For Pae
All men whove been diagnosed with BPH, who experience symptoms that are not controlled well by medications and who are not able to or do not want to undergo invasive surgical treatments, are candidates for PAE. Patients must have reasonable kidney function and not be allergic to the contrast dye used during the procedure.

Urolift Patient Reviews & Results
A review of 7 studies of 440 patients who had the UroLift procedure resulted in these findings:
All patients were over 50 with moderate symptoms. The procedure takes 50 minutes and half of the patients used local anesthesia.
Patients experienced a modest improvement in symptoms over 12 months of follow-up. The sexual function did not change. The results indicate the procedure is safe.
However, it was noted that those with very large glands and severe symptoms should consider using alternative therapies.

Overall Cost Of Urolift Procedure
The overall cost of the Urolift procedure should also include the cost of the evaluation. All patients interested in the Urolift procedure will need to have, at the very least, cystoscopy to evaluate whether they are a candidate for the Urolift procedure. Cystoscopy performed elsewhere will need to be repeated as it is a visual evaluation, and this 3-dimensional perspective cannot be obtained from a report of a cystoscopy performed elsewhere. A urodynamic test may also be necessary.
